I know exactly what yoy are talking about. When I replaced my rear shocks I took some vice grips and locked them down on the nut. That way when yo turn the bolt on the other side the vice grips will catch on something and hold the nut for you. but it is a real pain in the butt to get the vice grips on there, because you can't see it you just have to feel around and you can only fit one hand up there to do it.. especially on the side with the fuel tank, there you have to get under the truck and reach over the fuel tank to get to the nut on the upper portion of the shock.
I forgot the size, but find the size of the nut I thought it was 9/16, and buy a cheap pawn shop wrench, cut it off about 4 inches long and use the open end side, it fits on the nut and holds itself, while slowly removing it,it works great, and the length fits up there perfect. It is VERY hard to get the pliers up there to hold
I pulled out the inner fender liners on mine and it was not too bad to get at the top bolt. The inner fender liners are not the easiest thing to remove, but they do come out with a little persuasion.

Yeah it is hard to get those pliers upm there. LOL. Maybe a socket, with a 2 foot extension would works too.
