Default 97 caravan 3.3 4spd tranny slipping - Help!

Little help, please! Frantic wife called me up -van drives little ways then stops (transmission disengages).

Limped it home in L. Here's what I found:
[ul][*]Have first gear only.[*]Put in reverse =nothing[*]Put into D or 3= drives fineuntil approx. 15mph then upshifts into nothing (engine revs, no tranny engagement) - have to restart to get trans to engage in first again[*]Put into L (first)=drives[/ul]
Fluid not pristine, but not burnt, in hot range on dip stick.
No funny noises, no metal flakes on dipstick
OBDII codes: P0731, P0732, P0734 (gear ratio incorrect)
Cleared codes, took for test ride, no change, codes came back
Disconnected battery (heard clears "limp-mode" condition) - no change
After reading posts in this forum, I'm under the impressionlimp mode is 2nd gear & reverse, so my first gear only conditionis not trulylimp mode?

125K on van, but transmission was completely rebuilt <2.5 years ago (including clutches, steels, ring, speed sensors, converter, bearings/races, top planet, drum, carrier, filter, fluid).

What could be the problem? Any advice, troubleshooting or repair tips would be greatly appreciated!

Thanks!

Can you ck CVI ' S and perform a clutch test. Overdrive cvi's specs 75 to 150. if it over the specs a snap ring may have broken. Or maybe a hub. If hub breaks you will not hear any grinding noise and you will loose reverse.
