Default O2 sensor problems and where is it?

My van has been running like crap for a while. It is blowing white smoke when we idle (not head gasket, we checked that) it gets poor gas mileage, it stalls out sometimes, and I am getting the code for the O2 sensor. Does it sound like those problems could all come from the O2 sensor? Also, where is the sensor located? My van is a 95 caravan 3L V6. Thank you in advance for your help.

It should be sticking out of the exhaust pipe just downstream of the exhaust manifold.
