anti-freeze leak

can someone tell me about replacing the metal lines that travel from frt heater core along firewall and dive back under to rr heater core. I have a 2000 grand caravan with 3.8 ltr and the pipes are rusted and leaking.

you buy some new lines, then take out the old lines, install the new lines and top off you cooling system. How much more info do you need?
