It may be the rear heater core lines.

Is the coolant level dropping?
What color is the fluid being leaked?
What does it smell like?

The only "water" type fluids would be;
- Coolant/ anitfreeze
- AC condensation
- Window washer fluid

With rear heat and AC, there hard lines running under the van to the back.
The washer fluid also runs to the back.
Has to be one of them.

Best to jack it up (chalk the wheels and use jack stands), and get your drop light and take a close look.
Monitor the fluid levels closely.

Good Luck,
EZ1

The hole in the muffler is to drain the water so it doesn't corrode. They can drip quite abit and it is a common leak complaint.

When you have time, take the van for a spin and then let sit.Watch and see if the water dipping from the exhaust or the rear coolant hose's.
