p0016. Flex plate not cracked.

I have a 2005 Caravan with a 3.3L. I have a P0016 code. I replaced the cam and crankshaft position sensors. Light will only come on when I drive it. Won't go past 3500 RPM. I took off the transmission plate and closely looked at the flex plate multiple times for cracks and missing teeth. There is no crack nor, any missing teeth. Can anyone tell me what this might be. I'm at my wits end!!

When Monitored and Set Condition:

When Monitored: Engine cranking and Engine running
Set Condition: Powertrain Control Module detects an error when the camshaft position is
out of phase with the crankshaft position. One trip fault.

CMP WIRE HARNESS INSPECTION
TONE WHEEL/PULSE RING INSPECTION
CKP WIRE HARNESS INSPECTION
TONE WHEEL/PULSE RING INSPECTION
INTERMITTENT CKP SIGNAL
CAMSHAFT POSITION SENSOR
CRANKSHAFT POSITION SENSOR
TEST ACTION APPLICABILITY
1 WARNING: WHEN THE ENGINE IS OPERATING, DO NOT STAND IN A
DIRECT LINE WITH THE FAN. DO NOT PUT YOUR HANDS NEAR THE
PULLEYS, BELTS OR FAN. DO NOT WEAR LOOSE CLOTHING.
Ignition on, engine not running.
With the DRBIIIt, read DTCs and record the related Freeze Frame data.
With the DRBIIIt, erase DTCs.
Start the engine and run until operating temp is reached. (Closed Loop)
If the DTC does not reset it may be necessary to take the vehicle on a test drive.

Fighting this one myself on a 2000 Grand Caravan 3.8l. After over one year, found yesterday that the flexplate, which looked great from visual inspection, had cracked all the way around the hub behind the spacer, rotated a few degrees and locked up again enough to run another 10,000 miles. Only visible upon transmission and flexplate bolt removal. Symptoms: sluggish on acceleration from stop or uphill, intermittent ignition dropout on acceleration, intermittent P1391 code (crank and camshaft sensor disagree)/check engine light. Had the transmission overhauled several months back and specifically asked him to look at the flexplate. He said it appeared good from a face on inspection. I wish I would have asked him to remove it and look at the hub... Hope this helps someone out there
the only way to see if the flex plate is cracked it to remove the transmission. just removing the cover shield won't let you see the plate were it cracks right next to the bolts.
