First thing you need to do is update the PCM because the software in your PCM with not run a evap test.Ck all your hoses for cracks, if they okay then you could have LDP pump or vent valve that are bad.

One other suggestion. I had a small vacuum leak detected in my 97 caravan. CEL came one and stayed on. I went over everything, all the lines, could not see anything. Then I thought about the fuel filler tube, which, in my search for leaks, I noticed that mine was very, very rusty. On one weekend, I removed the fuel filler tube. Wired brushed the rust off, acid treated with rust mort. It was peppered with small holes and a few larger ones. Coated it with POR15, re-installed it, and the light went off and stayed off. That was three years ago and before they made the aftermarket fuel filler tubes. OEM from the dealer was $344, junk yard find was $50, and the aftermarket (now) $70. So check out that fuel filler tube as a strong possibility.
