I really don't know much about AC, so forgive any stumbles in describing this...

My 2005 Grand Caravan has driver/passenger split temperature sliders (not digital) and rear vents with separate fan controls.

The AC is blowing cold through the back vents, but seems to be blowing vent air through the front vents (both driver and passenger sides). The defroster doesn't seem to be cold, either.

Any guidance?

Could be the front h-valve stuck, it could be low on freon charge, could be a control head issue. You'll need to check the system pressures first and go from there.
