dodge caravan 2009 fuel efficiency problem

Do you measure mpg's per trip computer (it's way off often) or by measuring real fuel consumption between pump refills (from the full tank till the low fuel light lights on)?
Also, what is your driving style - if it is normal (no hard accelerations, no heavy traffic e.t.c.) than 10-12 is really low - it should be around 17 mpg.
Did you check the brakes (including hand (foot) brake) after 5-10 miles driving - if they are very hot (possibly even red!) than maybe they are drugging your mpg's down.
Another thing is to check spark plugs and their wires (you can see it in the dark garage or at night - open the hood and ask someone to rev up the engine and look at the engine area - if you see flashes of sparks - that means your wires are not isolated and need to be replaced).
Oil and air filter could also affect the mpg's - but not that much (like 5-7 mpg's - usually 1-2 mpg, not more). IMHO

I was getting 13mpg on my 3.8 las fall for a month, and I drive 60%hwy and 40%city,
my poor mpg was due to my fast accelerations from the stop lights, and long idle times(2 15min idle times per day)
as soon as I started driving a bit slower off from stop lights(keeping rpms at around 2500) no problems since then other then my high idle times, I get around 18mpg

Where do you live? an as mentioned, you using E85?
I am getting 17-19 mpg City and 38MPG highway. on my 4 banger Toyota Corolla. because i live in metropolitan city and drive like a mad man. 12-15mpg in metropolitan city is consider Normal for ur van. Our Taxi Cab gets like 9mpg.
So, might be normal for you.
