unfortunately it is part of the whole assy.
they are fairly cheap in local junk yard. Dodge minvan are easy to find. Probably $50-100 for used one.
when it comes to things like this, Used one is fine. I hardly seen switch fails, and they not wear and tear item.
If it is tested out bad, maybe you can try taking it apart and see if you can find any broken contact point you might be able to fix.

 

Had the same problem. My fan switch worked on all but high. Changed
the resistor first because that was the easiest to get at. Was lucky,
it was the resistor. However, if that doesn't cure your problem, there
are a number of temp controls on e-bay in the 35-50 dollar range. Also
you can get instructions on the internet to remove and install these.

Sorry for not updating the post as to what fixed mine. I was convinced in my head from what I read that I needed either a new temperature control and/or motor. But since I could get a resistor really cheap off Rock Auto I bought one and installed it. I thought if I am replacing all that stuff I should replace the resistor as well. Well to my amazement changing the resistor fixed the problem. The blower now works on all speeds as it did when I bought the van.
