I clean at 60,000 miles (95000 km). Some clean them when they do the tune up.

The throttle body needs to be removed and wiped clean with a rag with some solvent on it. Do not spray out a throttle body that is drive by wire as the cleaner can get into the electronics and mess them up. Torque is 50 in lbs in a criss cross pattern. If it is in the shop we would also perform the throttle body relearn procedure. If the throttle body wasn't real dirty, probably not needed but if dirty, might get a high idle.
