When a vehicle is first started after the pcm is reset it takes a min to reset the idle (aka takes a while to fall back to idle), If the pcm's memory is shot, that could be the whole issue, it's not saving the idle setting, meaning every time it drops back to idle it has to relearn where it should idle. It makes perfect sense, it just finally lost all memory for the idle and has to reset every time.
