Vehicle: 2002 Dodge Caravan (3.3, V6)

The driver door power window glass keeps falling off its track. I've opened it up and reset it, put the clip in place and it works fine but after so many uses it pops off again (specifically the front most glass lift plate notch seems to pop out of the clip. the back glass lift plate is still attached to the regulator clip fine). All clips appear to be fine. Is this just a problem with the clip OR something else? This last time the window popped off the track after i lowered the glass (all fine) but then exited the car and closed the door; when the door shut that is when the glass popped out.

The only oddity with the door is the door isnt sitting straight and so scrapes when opening and requires a little extra push to close it.
