Default Need help with some codes

Well, today I had my first issue with my truck. Here's the back ground.

Filled up my truck last friday with 30 gallons of fuel from a place that I always go. This weekend, I helped the inlaws move up north. So today, I pick up my dad's bobcat trailer, top off with 9 gallons of fuel and continue to their house. We get loaded up and take off. About 10 miles into the trip, under easy, steady excelleration, the truck surges a little and misses, then dies.I pull over, turn the truck over, it starts, then dies a second or two later. I give it a minute, start it up and it idles fine. At this point, I'm thinking I might have got some crappy fuel at the last place. I had the previous tank blended with Power Service (white bottle,) so I figure I would be good.

I get about another 20 miles away and it starts doing the same thing. I was going about 50mph and it starts shuttering and skipping. I pull over into a parking lot and from there, it will start, idle for a second and then die. My father-in-law had the same thing happen to him the other day, so he threw some Power SErvice 911 in and it took him an hour or so of turning it over, getting it to start and idleing for a few seconds before it cleared itself out. It's ran great since. THinking that was my problem, I got a bottle of 911 from the gas station and threw it in. It would continue to start, but then die right away. I changed the fuel filter, try again but no better. It gets to the point where it won't start. I check for codes and get the following:

P0628 - Fuel Pump Relay Control Circuit Low Voltage

P0483 - Cooling Fan Rationality Check Malfunction

P2509 - ECM/PCM Power Input Signal Intermittent

I look up the codes on a buddies phone quick and the only one I found was P0628. When I saw Fuel Pump, I quite trying to start it. I had it towed to a buddies heated shop and left it for the night. We've been getting to about -15F with highs around 10-15 degrees.

Any help on what all the codes mean and what to look at? Thanks,

your lift pump may have died and one of your batteries is going dead.

Was there any white smoke when the truck was not running right?

Thanks Drew...

I was watching for smoke and I didn't see anything. When it did start back up the few times, it didn't blow any smoke and if I stepped on the gas, it would just blow a puff of black smoke.

Anyone know what this one deals with?

P0483 - Cooling Fan Rationality Check Malfunction

Most likely going to be making a purchase from Glacier Diesel in the near future and looking into some Optima Batteries. Just what I wanted to do a few weeks before Christmas......

Thanks again for the help

I would worry about the other two before the 483. . .it could be the condition of the batteries that caused that
