Unhappy '06 cummns overheating at idle

We have an '06 3500 newly, rebuilt tranny, new fan clutch and an overheating problem at low rpm and idle. Temp goes to 220 at intersections, stop and go traffic. We pull a 53' trailer/haul cargo trailers. Replaced coolant temp sensor as well. Temp at highway is 195 - 200.

Any advice re possible problems would be greatly appreciated!
