You might have a broken motor or transmission mount. If one of those is broken the motor and tranny could be moving around and forcing the shift linkage to slip the tranny into neutral.

 

I hope it is the motor and/or tranny mount. I have(mine is only when below freezing tempatures) a simular problem with the transmission in my old Impala and it turns out to be a internal problem. The only way to fix this if it is not a broken motor/tranny mount and the gear selector is not being moved into neutral (where you have to pullthe selector handleback into the "D" position) is to have the tranny rebuilt. If you are having to move the gear selector back to "D" position when it happens then as Alloro said youshouldfind abroken mount.

Motor = your engine
Tranny = your transmission

The mounts are what hold the engine and tranny to the van's frame. With out them the engine and tranny would fall out of the van.
Your engine has 2 motor mounts (side of the motor towards the middle and bottom of the block) and the tranny has one usually towards the back of the tranny just before the drive shaft.

 

I am sure you are not silly nor stupid. Your english is way better than my Italian, for I do not know Italian at all. I wish I did know some of the Italian automotive terms where I could help you understand your van better. Anyway I am glad to help the best way I can.
