Is your gauge showing that the engine is coming up to full temperature? If not replace the thermostat. If the engine is coming up to temperature, then either your heater core needs a flushing, your coolant is low, or the blend door isn't moving.
For the blend door, remove the glove box and the blend door motor is right there. Start the van, turn the heat on, then turn the temperature dial up and down. Watch for the motor to move and the linkage rod running to the blend door to move.

The heater core is under the hood on the passenger side, in that big black box. If you look back on the firewall you'll see the two heater hoses going to it. To flush the core you would have to disconnect those two hose from the core, then blast water through the tubes the hoses came off of. At least once in each direction.

With a marker, pen, pencil, or whatever works...mark the position of the linkage rod in the motor arm, then disconnect the linkage from the motor. See if you can manually move the linkage and get hot air to blow out of the vents. If you can't move it, your blend door is damaged. If you can move it and get hot air, then you'll next need to pull the wire connector off the blend door motor and check for power there (key and heat system turned on). The power should vary as you move the temperature **** from hot to cold and back again. If you get the power readings like I described, then replace the blend door motor. Otherwise the problem is either a blown fuse or the temperature control on the dash is no good.

Um, this is probably a bad time to ask, but what year is your van?
