Wife causes van problems
Could be a coil pack I suppose, only does it on cold engine starts for like 2-8 seconds. Happens so fast and is over so fast that it's hard to catch, and the plugs read fine- nice creamy tan color. I would think if it was the harness or grounds it would do it more consistently or over bumps.
My train of thought leads me to believe that it relates to initial engine temperature at start up. I would like to think that it's because of the lack of catalytic converter, but as we all know, open loop ignores the 02 sensors on initial start up. I am going to install a cat, as it should have one anyway, but I doubt it'll fix the problem. Next place I will go is a drip test on the injectors. Perhaps they are leaking fuel while the van sits. I will also try and borrow a live feed scanner to read coolant temp on startup.

Since only on cold starts that this happens it sounds like your PCM may be on the verge of going out. Try the hair dryer trick before the first start up of the day and see if that helps. If heating it up for 10 minutes causes the start up misfire to go away then that would be the clue to look for a new PCM.
