Default 04 R/T dangerously loose in the rear end

I've only had my R/T for about a month and I've noticed something. Everytime I hit a bump doing over about 60, the rear end gets squirrelly in the rear end. Driving to work on the highway yesterday, hit a bump on wet pavement and ended up facing north in the southbound lane.

I know I probably need to replace my rear tires, but is there anything else that could be causing this? I'm honestly scared ****less about whats gonna happen come winter time with snow and ice.

You may want to have the car raise and check the suspension, shock or struts as well the tires.
