Default 360 Revamp

Alright, I have a '94 Ram 4x4 with a 360cid. I have 148k miles and I want to make my truck run like i know it should. First a list of little problems i haveuring engine warm-up, my truck cuts our pretty bad and this continues now until i drive a mile or so; the transmission fluid doesn't seem to start circulating until i drive 2 or 3 miles(when i first shift into drive, it takes a minute or so for my truck to start rolling, without pressing down on the accelerator at all; during idle, my rpm's will jump up and down about 100rmp.
Now for everything i have already done to the truck:new transmission about 10k miles ago; new dist. cap, rotor-bug, wires, plugs, and coil about 2 yrs ago; mac performance air intake about 500 miles ago; took off throttle body and cleaned out very well about 600 miles ago(it sounded like a vaccum leak, and i found out that my stock airbox was letting dirt get through and dirt was caked on the butterflies!); new radiator about 2 yrs ago; new thermostat, radiator cap, and lower radiator hose replaced tonight; new water pump lat year; new belt last year;
I have one 40 series flowmaster dumped, with no cat(y-pipe), i still have the stock exhaust manifolds
I have used mobil 1 10w-30 since i bought the truck from my grandpa in 2001 with 93k miles(he bought it new in '94
i usually run 87 octane fuel because i only get about 12 miles per gallon, which is better than 10mpg i used to get before i took off my cat.
I want to get the motor running good because i want to start trying to get more hp out of it. IF ANYONE CAN OFFER SOME SUGGESTIONS I WOULD GREATLY APPRECIATE IT!

If it were me, I'd warm it up and do a compression check and leak-down to see what kind of shape the engine is in. 148K is a lot of mileage as you know, so you need to make sure you've got a good engine to work with before trying to mod it.
