Alright, I am pissed...
This is the second time this bull**** happens to me. I'm driving my car, chilling, messing around with an Evo, then I feel my car different, I dont hear the flutter from my engine bay from the AEM intake system. I get off the freeway, pull into the gas station and low and behold my motha effin tube is disconnected again. WTF MAN! I tightened that bitch up real tight. This is starting to really **** me off. Any suggestions? Anyone else had problems like this>
For starters, try a different clamp altogether. Secondly, take the intake completely off the car and inspect it. If the outside of its been damaged or has been stressed, it may be time to consider a new one..
Also, is it properly supported? If it is, is it being held too tightly? Parts being tightened too tight can vibrate loose and fall off.
To be honest, I almost want to say it may be time to let a shop look at it. Or find someone with more car know how then you. You can't torque things too tight.

With hose clamps, if you tighten them too much you can strip them, making them virtually useless. Head down to your local auto store and pick up a 3" hose clamp, or a couple for that matter. Replace the old ones.
I've had my intake on since December of 2004, and I have NEVER had a problem with it falling off, its an AEM SRI.

I'm wondering how you have it supported. If you have stock engine mounts, the engine moves a LOT! The whole intake has to be able to move with the engine. If you got the filter end strapped down hard to something stationary, maybe that is the problem. Maybe you dont have enough flex in the system.
