95 Water in headlight

So the passenger side headlight has a puddle of water and both headlights have condensation. I was wonderin why my headlights looked so crappy
I had to drive with high beams on to make it home in the dark.
What causes this and whats the right fix for it? I searched around on the interenet a bit but couldnt get a straight answer.

Its pretty cool. There is like one tube on top facing down (as a whole for air to be sucked in) and one on the bottom facing down (I assume for air to go out or condensation)
The bottom tube for some reason wasn't letting any water out so I just yanked it off and all the water changed out. The wholes are there to allow for the pressure change the heat of the bulb causes. Since it was the bottom one no rain will get in and the top tube till can let air in and not rain.
