Is this a misprint? No filter on a NV3500 I assume it is. Also, what fluid did you use?

If you have the correct fluid and its full, its more then likely the front input bearing.
I used Mopar Manual Transmission Lubricant 4874464 as per recommendation by allpar.
It is sounding like the input shaft bearing then. It's been making the noise for as long as I've had the truck. Is it something I need to fix ASAP?

I agree, sorry, but no way to know how long it would last or how much more it would tear up. FWIW I have read where rebuilding the nv3500 is pretty involved. Needs a lot of special tools. But on the other hand, I have read to pull the input shaft and bearing is not too bad of a job......
You may also want to consider running it until it gets worse, then buy a reman, or a used unit to replace it with. If you decide to go used, make sure the donor does not have the electric shifted transfer case. On those, they took the bosses off of the side of the transmission that mounts the transfer case shifter. I have found that out the hard way.
