I have been researching this same problem.
The part is dealer made only and is called REAR HEATER HOSE 01-03 OEM 55056060AC. It runs between 60-70 dollars online. Here is a picture attached of it.
Another solution is to make your own part with a Y-connector and clamps. I read and was told I could get a brass plumbing Y-connector. The internal diameter of the heater hoses is 5/8 inch.
An additional problem - if you log into auto part stores online and input your car make,model, year, then what happens is no heater connectors will appear as compatible. However, all you need is a universal heater hose Y-connector with a diameter of 5/8 inch.
I was able to use this part by Adelco 15-31767 Heater Hose Nipple (Y-heater hose connector). There is a photo attached of this item. I also found an Adelco pdf which lists the part on the last page at http://www.acdelco.com/pdf/appcat_bu...e_fittings.pdf
I could not find the Adelco Y-connector at any of my local auto stores so had to order it online for $10.00. With clamps from a local store, total was $15.00.
My cousin mechanic, sliced the original Y-connector off from the other hoses. He used an utility knife. (My leak was at the Y-connector). Then he inserted the adelco Y-connector and clamped the 3 hoses. Works great.
