Default Running problems

I have a 89 dodge d100 and when I start it it runs ok , but when i put it gear it gets real choppy and shakes really bad amd barely wants to go. It'll move but when I press gas it dont go no where for a couple seconds and then will catch up and drives horrible. I had the problem a few weeks back and i cleaned throttle body and ran fine but just the other day when i was driving it started to tug and start sounding choppy again. Withing last month i have changed - coolant temp sensor, distributor rotor and cap , ignition coil , spark plugs , cleaned throttle body, IACV and put injector cleaner in gas. Any suggestions
