a boot with two clamps, a section of 3" PVC pipe, about two feet should do, a couple of 90* elbows and maybe a 45*. Some PVC Cement, paint and maybe a small rubber grommet if you have an air sensor that needs to be plugged into it.

had no trouble at my local Home Depot, but I've also bought everything I need at Ace Hardware.
The tube is a breather tube for the crankcase. You can either get one of the little tiny filters made specifically for it at any auto parts store for about $8, or drill a hole into the PVC of the intake slightly smaller than the diameter of the hose and stick it in. In the second pic of my above post you'll see the breather hose filter I stuck thru a hole I drilled in the stock air box.
This is one I made to send to a gal who has a 2500 Hemi 4x4. The hole with the rubber grommet in the middle is for her MAS, if equipped you need to get a grommet that the sensor will squeeze nice and tight into. Again, Home Depot has them for about $1.49 for a pack of four...

In this one, you can see I went very close to the sidewall and then back out with a 45*, to accommodate the filter. This is because this CAI is to replace one she got off of fleabay that was very cheaply made and broke on her. I had to go close to the sidewall to catch a bracket as she no longer has her stock box.
In her case, I used an aluminum shelf bracket that I put a piece of adhesive foam on so as not to scratch the intake. It's unpainted as she wanted to do that herself...

i guess i can chime in now. mine is a 4.7 but the engine bay is near the same.
starting from the tb i used a 3"-3" rubber coupling a small 3" dia tube @ 3" long
then a 90* piece into a staight piece about 10-14" long, (check urs b4 cutting)
last piece i used was a 45* street elbow w/ another coupling on it for the filter to slip in. after u get everything setup the way u like it cement it and paint if you like.
and i put a little filter right on the breath hose.
