Default 97 5.2 dakota won't shift out of 1st
My truck doesn't want to shift out of first gear. Sometimes it slips or grabs really hard. I tried changing the filter and fluid. What else should I try?

Is the fluid at the correct level? It sounds like your 1st clutchpack is toast. Did the fluid smell burnt or was it black when you changed it? Your probably looking at a rebuild/swap.
