Why other type of regular maint. have you done? Plugs, air filter, position sensors etc. Have you done the ignition key, 3 off-on-off-on-off-on to see if you have any codes possibly? Hesitation can be a number of easy fixes we've all had them here
don't add oil cleaning agents to remove sludge it will just swell the seals a bit and eventually start leaking from them, best advice is regular oil changes.

Clean the throttle plate is not very likely to fix your car hesitating problem.

p1684 can a be a few things, like if the battery cable was disconnected to clear other codes which never helps. Only pull the pcm/fuel pump fuse to clear codes after repairs or just do simple pcm resets.
P1684 means the PCM has lost power while the car is off. Your battery posts may be corroded or a battery wire may be losing it's connection might check post cable for corrosion & your grounds too.

It sounds like some new spark plugs and wires are in order, to me anyways. The plugs should be changed every 30,000 miles and the wires every 60,000. How to remove sludge? Either do a few oil changes rather close together, or if that doesn't work, try some Auto-RX. Do you know why it has sludge built up?
