The diaphram itself cannot be purchased. You have to either find an old CAD at a junkyard and pull it apart, buy the whole CAD new, or replace it with a cable actuated posi-lok.

There ain't much to map out.

Transfer Case - Big thing behind the transmission with 2 drive shafts coming out of it and a shifter linkage up top going into it via 2 rails.

Front axle- Very simple due to full time wheel to axle engagement (AKA no locking hubs). You have a vacuum line running to your CAD and that should be all you have to worry about without a major axle tear down.

Basically we need to answer Ice_Man's question, what isn't engaging, the t-case or the front axle? If you can pull your lever all the way back and feel it hit a little detent, then the t-case should be enaged. Are you hearing gears grind or what?

Edit: If it weren't snowing here, I would crawl under my truck and take some pics and label themfor you, but sorry, I ain'tlaying down in the snow before class...

If you go with the posi-lock as zique mentioned, here's a link I found that I have bookmarked for when I get mine fixed. With all of the problems with the vacuum lines that I've found on mine, I am not even going to worry about whether or not my 11 year old vacuum actuator on my front axle is still working properly since I can't really determine that until I replace all of the missing / damaged and plugged vacuum lines. I don't really need the 4wd so this project is on the back burner for now. Here's the link:

haha...sorry, I coulda swore that said diaphram when I first read it. There is a diagram on here somewhere. Do a search for CAD on here and it will come up. At least I found it when I was looking into a posi-lok earlier this year.
