With the engine off try to spin the fan by hand. It should have resistance but be smooth and quiet. If it spins easily (like all the way around or more) with a slight push or is noisy you need a new fan clutch.
Did that, it spun easily and was noisy. Truck is at my mechanics right now getting them both replaced. Hope it doesn't break the bank.

Mine has also made this sound the day before my birthday. turned out i was having problems with overheating in this make, model, and generation. mine is due to the Fan Clutch. I also replaced the water pump just in case. right now as of June 20, 2008 I have found out the replacement is on National Back-Order. The part is currently $194 and is Part #52029876
