have you changed the output shaft speed sensor? i had the same problem a while back, truck used 3rd gear and higher, changed the sensor and all was well.

My truck had the same problem. also an 01 1500 w/ the 5.9. Started in 3rd when the truck was in drive but I could manually shift it to 2 and 1 if needed.
I was pulling the 1756 code. I replaced the governor solenoid and transducer and its working fine now. Cost about $150 and 2 hours and I was all good.
