What my friend did on his 01 1/2 ton was had his ball joints chisled off because they're riveted on to the A-frame.

only if its 2wd. if its a 4x4 they press in.

Depends on what model year, some pressed, some tacked, some riveted.
My 96 Indy (4x2) has pressed in balljoints, put in moogs few months ago

Can't get the upper ball joint out with this puller/presser thing...

Out of the control arm? Or, out of the spindle?

If the spindle: Get two big hammers. (one handers) Hold one against one side of the ear the balljoint goes thru, and HIT the other side with the other one, just as hard as you can manage. Couple good wacks, and it will pop right out.
