I have a 2001 Dodge Ram 1500 Laramie SLT 5.9L V8 4x4.
Driving down the road at about 55 to 60 miles per hour and since I have custom exhaust on there I can hear it surging a little bit every once in a while and the RPMs will be at about 1800 and spurt to 2000 when it does it.
Any ideas just started happening so any help would be great before anything gets worse.

With nothing more to go on, it sounds like your torque converter is unlocking. Does the instantaneous load on the truck (hills, wind) seem to correspond to it, or is it totally random?

Get the pcm flashed. Sounds weird, but when they get old, they "forget" things. Exact same thing happened to mine (torque converter would lock and unlock constantly at highway speeds), and that's what fixed it.
