Red face Plenum failure?

Since I've been looking around here the past few days I've noticed a few references to "plenum failure" . I haven't worked on many Dodges but did mechanic work for a few years in the late 90's. We didn't see many Dodges. About 10 Chevys to 7 Fords to, maybe, 3 Dodges. So I guess what I'm asking is: What are the signs? Does it happen around so many miles or any time? How easy of a fix is it?
