Would be interesting to see if you had any pending codes.

I am fairly certain it's an engine problem. (as sure as I can be without actually laying hands on the truck.....) Yours is behaving exactly the same way mine did when I had two dead cylinders. (plug wires routed wrong, my bad.....)
