griding noise

Got a 95 dodge ram 1500 5.2l 4x4 with the nv3500 transmission(manual) anyways my truck has a grinding noise only when the truck is under load it doesnt seem to happen
in first gear... in second gear is were is kinda starts with a subtle noise of grinding then when im in 3rd 4rth and 5th you can hear the grinding good. also the longer i drive the truck it seems to get louder when its warmed up.
Things i have done so far...
changed the tranny fluid(synchromesh)
changed the transfer case fluid (ATF+4)
changed rear drive shaft u joints.
things i havnt tried is checking the CAD(center axle disconnect) vacuum. 4wd light does turn on though when i put in 4wd (probably gonna get a posi lok even if that not the issue just good to have).
I noticed some other forums said it might be something in the trasfer case like some sort of snap ring.... havnt checked if its broke.
maybe its just a bad tranny? or bad transfer case?
If you guys have any ideas that might help it would be great i really want to get rid of this grind. i know its hard to diag over a forum but i appreciate any advice given!
