Thanks for those photos. It helps to better quantify your issues.

The kickdown lever needs to be installed onto the top stalk of the valve body. It's missing a bolt and nut to secure it to the valve body stalk. As for the bell housing cover gap, I'm not sure what to make of that, but it won't hurt anything.

Depends on the vehicle. My 98 durango had the cut out on the bellhousing and matching dust cover. The donor trans didn't have the cutout and was from a 2000 ram. I ended up having to swap valve body because the electronics switched in 2000.
