When it doesn't start, do you hear a "click" sound?
If you do, its the starter solenoid going out, and you better replace that starter before it completly dies out on you.
Mine did the same thing (sometimes it would start first try, other times it would take a few tries) and it only gets worse.

Sounds like it is time for a new starter to me. I bet if you replace it, the problem will go away.

I had the same kind of trouble with mine on a Dakota, around 215,000 miles. I found a rebuilder and just bought new contacts and brushes from him, cost me under $20, substantially less than a new starter. He told me Denso starters are pretty solid otherwise and I shouldn't have problems with it after that. The contacts that came out were almost worn through, I was surprised it worked as well as it did. Also when you're down by the starter, clean the end of the cable so you don't have problem with voltage drop.

ya mine went about a year ago same symptoms, they're a pain when they don't work properly. You can try apply the brake and shifting into D and then P then trying to start again, aligns the starter with the flywheel gears.
