Hemi with MDS
I know the MDS saves a little gas, but my averages are pretty close to the 05 and 04 Hemi's I've seen guys post. If you're getting 15 combined city/hwy driving, that sounds good to me. My truck is a 2wd, and I average right at 16mpg with 80% highway / 20% city driving...

Thanks for the info bwhite. I have found this a bit confusing, because the service manager at the Dodge dealer where I bought my 2004 truck told me that a few of the later built 2003's were introduced to the MDS hemi, and that 2004's and up had them, but that seems contradictory to what others have said. I have been trying to see if there is some way I could find out more specs on my truck such as axel ratios etc. but have not had much luck so far. I have checked my mileage often and average 14.9 in-city driving and about 17 hi-way. Not too much different than my old F-250 Super Duty with 5.4 L V-8. My Dodge does seem to have much greater response with the 5.7 L though.

Well it's well documented, but MDS for the Hemi was not introduced on the market until the 06 models..
Dodge trucks are not known for good gas mileage, I'd put money on it that GM trucks have the best mileage... I could average 20mpg easy in my old 02 Sierra, and my company's Avalanche that I drive quite a bit gets a whopping 23mpg on the highway.
However, if your numbers are accurate, you are getting better gas mileage than most on here. 14.9 in town is a really good number, my truck gets around 13, highway I can see 19 as long as I don't hit too many hills, usually it's 17-18 though, and I've got a couple of mods that help out MPG.
To answer your question above, there is no aftermarket MDS upgrade. If you want the MDS, you'll have to get a new truck.

my 04 5.7 QC 1500 gets 14 city and 19-20 highway doing 65 on cruise control, i always thought i had MDS, my brothers 03 5.7 QC 1500 gets 21 highway at 65, he has a K&N CAI and some 2.5 duals with 3in tips, how come we get the same mileage as the MDS trucks? thats crazy, i swore up and down i had MDS
