Default Reverse slipping out

So this has happened to me once and my girlfriend twice now. backing the truck up it slips out of reverse and goes to neutral. now she also noticed today when it happened that the indicator box disappeared around the R, and the indicator box was not around the N either. but it was in neutral. Havent checked the fluid yet, but going out the door right now and it will be the first check. if the fluid is good, does anyone have any idea what may be causing this? Thanks for the replies in advance.

Sound like the shifter might be slightly out of adjustment and not fully in the reverse detent.
