Question Strange noise at idle and throttle increase

Looking for ideas on 04-2500-slt 5.7
I started the truck the other day and I am getting a very odd noise like two gears trying to mesh around lower front end, the tone does not change with increase of the throttle but does follow it and gets louder.
Put it in gear makes no differance tried to engage 4L and high both are working.
Definetly not a belt sound happens standing or moving??
Seems to be near the oil pan, tried to feel around for odd vibrations did not notice anything really drastic.
Not sure if this matters the truck is not driven regually
Does it also in all gears as well as Park

Any thoughts?

Try removing the fan belt and starting it for a minute to see if the sound is still there.
