Well, after a week at Dodge, a thorough top end cleaning did not fix the problem, it was a bad injector. Will be picking up the truck tomorrow. I believe the bad injector was causing the low idle, I asked them to double check, will find out more tomorrow.
Let us know, because you're describing the same thing on my '04 and it seems like I've done just about everything for it. I've been putting off injector testing since it's rare that I see injectors causing a problem with anyone else, but if it turns out it's your problem, I'm very interested.
500rpm at a hot idle in gear is normal so don't be surprised it it still does it. It shouldn't die or be real rough though. Mine has done it since day one with no ill effects.

So far, my idle hasn't dropped below about 600-650, been idling perfectly. If anything changes, I will post.
