Sounds dealer supplied...factory supplied would have it installed and fully integrated into the electrical system.
Sorry, the harness plug was included and in the glovebox, not a controller - I wasn't writing very clearly....
I bought a Tekonsha Prodigy that year... pretty good unit, still have that too. I've always just tagged onto the bolster bolt(s) at the bottom of the dash. That way when I sell the truck, no ugly holes.
