Default 1998 caravan surging problem

I have a 1998 caravan that hesitates and feels like loss of power when i take off from starts. It jerks and it feels like its a fuel problem but i took it to a mechanic and fuel pressure is fine and I replaced the pump and filter about a year ago. The error code for bad cam sensor and crankshaft sensor came up so the mechanic replaced them. It ran fine for about 3 weeks but now its doing it again. At first it would only surge when the gas was below half a tank but now it does it all the time. My gas gauge also doesnt go all the way down to empty, it will run out of gas at about a 1/8th of a tank. Any ideas of what the problem could be?

Probably the harness for the crank sensor is rubbed through and shorting out

Could be the wiring like tizzy had said, but i going to add maybe the flex plate? You could also have a problem with the sending unit. Also should have the gauges calibrated.
