Default 2002 Dodge Caravan 3.3L V6 stalling

Recently my mother told me that her 02 Caravan is having a stalling issue. It has around 152K on it. She was having problems stalling problems before her 150K service and she had the plugs and wires replace, the transmission fluid flushed, the coolant flushed, & oil (naturally). It seemed to run better; however, it started acting up again. It stalls when she starts to stop. The rpm runs below what it is supposed to and unless she puts it in Neutral and revs it, it will stall. It will fire right up though after it stalls. My thoughts of what it could be:

1. The air filter needs to be cleaned as it was last done 30K ago (K&N)
2. The fuel filter needs to be replaced (as far as I'm aware it has never been done)
3. The vaccum (I hope I'm not talking nonsense here) might have an issue (I heard about this somewhere before and at one point the check engine light claimed that, but I thought we had that fixed)
4. The Coils might be getting old.

Any advise would be appreciated.

any one capable of doing diagnostics on the van?

If it was me looking at it, I'd drive it to verify the problem then use a systematic approach to pinpoint possible causes.

on thing that comes to mind is EGR

I would agree to the EGR,But i would remove and clean the idle speed motor first. And also have the van scan for codes.
