Go to the parts store and buy a 3/4 inch t connector and a piece of 5/8 hose and 3 clamps replace the hose going from the fire wall to the Y Connect the T cut the other hoses and connect to the other ports this will fix it. OR you can use a hack saw blade and cut most of the way through the plastic rings around the hoses and pry the plastic off (do not cut hoses ) replace plastic rings with hose clamps but do not overtighted this will solve the problem too

For those who used the T fitting instead of the Y how did you set it up as its quite hard to see by the pictures posted earlier in post, is it set up like this:

Or like this : or doesnt it matter either way? and any reason why a brass fitting would not work?

Glad I finally found this forum. Thought I was going crazy trying to locate this stupid part with no part number. I'll be heading to store to get parts & fix this tomorrow. The Y is totally cracked on mine and I used JB water weld to patch until I could get it home to do the repairs as my van was overheated, out of coolant and stuck in a parking garage. Thanks for the tips and I'll report back. I agree this should be yet ANOTHER recall for these vans.

I have the same leaking "Y" issue here. The leak occurs at the seam between the connections at the top of the "Y", NOT at the plastic pressure clamps (as some have suggested in the many forums concerning this issue). The connector is obviously a **** poor design that cannot tolerate the stresses imposed by the 3 hoses, and the heating/cooling cycles. It is probably not even made of auto-grade material. I temporarily repaired it with JB Weld, and also bought the "Y" manufactured by "Four Seasons" for permanent fix. This issue should be a re-call, but is ignored by Chrysler because it is not a "safety issue". In any event, the part in question should have been replaced with a more reliable version years ago. I have alerted & provided detailed information to several heater connector manufacturers with regard to this issue, and asked them to consider making a replacement part in a kit form including a 3" hose & required clamps.

Thumbs up Thanks for the Coolant Leak Fix!!

I took the part # and photo that alintx posted to a local O'Reilly's Auto Parts store and about $30 later (3' of 5/8" hose, 4 1" clamps, 1 gallon of antifreeze, and the Y tube) I fixed the coolant leak myself! Thanks to alintx and those who posted notes about this fix!!

I just fixed mine today, $28.03 in parts, 15 mins, and a pair of pliers later puts $320+ back in the bank. Me my wife and 4 kids thank you all for your great info and direction, as far as the dealership I went to to see what's up with the van.....I say kiss my a$&!!!!! Btw while under the hood I checked the oil and well....3 qts down!!! I have 56,503 on this used van I bought 6 months and 3,000 miles later am running into all this??? Let the fun begin huh oh and I complained to the NHSTA and Chrysler while waiting for my recall service to get done

I had the same leak at the Y-connector on my 2008 Caravan. Dealer part and repair was way too expensive $300-400. So I just repaired it myself. I cut off the 2 outlet pipes right at the Y with a box cutter, removed the hose connecting the y to the engine, and replaced it with a T-connector. All you need is a 5/8" T-connector, about 5-6" of 5/8 heater hose and 4 small ring clamps. Total cost (not counting replaced anti-freeze) about $13.00. It took me about 40 minutes to do the entire job (including replacing antifreeze). Pretty simple job. Don't even remove the hoses past the Y from their connections on the other end. Works great.
