Default Overheating Engine & No Heat out vents

2004 Grand Caravan
Van has been overheating and coolant has been low. Filled it up (water) and the van overheated again. Checked the coolant and it was full. Turned on the heat to lower the engine temp, but heat would only kick in periodically...it would blow slightly warm or cold.

Any suggestions on issues would be greatly appreciated.

Pressure check the cooling system and inspect for leaks. No leaks, then I would check to see if the coolant fan comes on. You could have a bad theromstat or water pump.
