My van is factory EGR delete and it makes that exact noise.

Thanks everyone for all the input. Was planning to remove s-belt this wkend to isolate noise. However, I don't have the special tensioner release tool and none of my ratchet heads are thin enough to clear the body/frame rail where the tensioner is. Will get back when time allows. Vandad.

You can buy the kit which works great or grind down a 15mm socket so you can fit it and a breaker bar in that space.

Belt tensioner - changed. Noise still there. ((( Now they say that's alternator bearing. Will try alternator soon.

my '03 makes the same noise and the dealer diagnosed it as the timing chain. would cost 900.00 to fix. But the service tech told me not to worry about it as they usually last a long time (but the noise is annoying)
