I had this happen to me on my 05 ctd. If the truck was idling andyou stepped on the throttle to bring up the rmps the engine would take off to 3500-4500 rpms and surge. I took it to 2 dealers and they could not find the problem, they changed fuel filters, flashed the ecm, etc...
I had to drive 6hrs to a rep. dealer and they fixed if. They replaed all 6 injectors.
I am not saying this is your problem, but it my be something to look at.

I was reading on this a while back and the most common problem that I found was that the floor mat mashed the pedal down. Are you sure this wasn't the problem?

If you have a 94, 95, or 96 they had MAJOR problems with the throttle cable freying. It would get pulled into the cable's sheath, and then get stuck. It was a big problem back in the mid 90's

Good luck with your ride.

if I put my foot into the throttle on mine she'll hit 32-3500rpm then slowly back down. this is common on HPCR's since the fuel rail is highly pressurized with fuel and it takes a bit for the injectors to bleed this back down especialy under a loaded high boost condition.
