Default Cummins, Idleing rough?

Hey guys, just thought i'd throw this out there and see if anybody could figure out what wrong with my truck,
Problem, Idles rough, step on the gas peddle (in park) and remove your foot the idle stays high, put in gear it goes down. once and awhile when you start off it bogs down and then picks up? Hopping to fix this my self, if not its off to the dealer. I hate dealer everytime i go there my rectum hurts!!!!

PS, no mods, just 5" exhaust and air cleaner, 177,000. miles

i have a 04.5 and if i throttle in park it stays for a second or two then it comes down to idle don't really know why it does it just does then again why rev it up at idle in park?no point in doing that.
So it idles ruff hummm well is the check engine light on if so get codes and then talk back with us. Is it smoking black or blue at idle?
Also check the air intake hoses coming off the cooler they could be loose and letting the air get out. Check the cooler for cracks 05s are plastic end caps on the cooler and those crack.
when was the last time you changed the fuel filter? if running bio it does clog them up faster then using regular LSD or ULSD

Fuel filter is new, i use reg diesel fuel. Codes as follows; p2609 - p1740. Just got the truck back from trans shop today, (rebuilt unit w/tc)

after reading what Brian posted check your grid heater ground and power wires, batteries, battery cables take off each battery wires and test batteries seprate from each other. that away you get a good reading from both batteries.
and come to think of it my truck does stay at high idle if you step on the throttle when its in park. I have no idea what is wrong with that i'll ask around
